Registered Nurse - Cornwall

We’re looking for a compassionate and confident Registered Nurse to join our Cornwall Countywide Coordination Service, supporting timely access to the right end of life care for patients across Cornwall. This is a role where your clinical judgement, communication skills and end of life care expertise will make a real difference. You will help assess referrals, coordinate packages of care, support rapid discharge from hospital and work with professionals across the local health and care system to help people receive care in their preferred place, wherever possible. Hours: 27 hours per week Contract: Part-time Permanent Salary: £32,073 - £39,043 pro rata per annum, plus enhancements and a whole range of Marie Curie benefits. Location: Cornwall The Cornwall Countywide Coordination Service plays an important role in helping patients, families and professionals access the right care, in the right place, at the right time. As a Band 5 Registered Nurse in this service, you will use your clinical judgement to review and assess referrals, make decisions that influence access to care and coordinate support across a range of settings. This may include working with primary care, community services, hospitals, care homes, hospices, GPs, discharge teams, acute teams and other professionals involved in end-of-life care across Cornwall. This is a highly autonomous role, suited to a nurse who enjoys working collaboratively, making informed clinical decisions and supporting care coordination across the wider system. You will be helping to ensure patients receive clear advice, timely support and access to the most appropriate services for their needs. You will: - Use your end of life care expertise to assess referrals and identify the most appropriate care and support. - Make clinical decisions that influence patient access to services and help coordinate timely packages of care. - Support rapid discharge from hospital by working closely with discharge teams, acute services and community colleagues. - Provide clinical advice and guidance to colleagues across local services, with a focus on helping people access care in their preferred environment. - Work across primary care, community services, hospitals, care homes and hospices to support joined-up end of life care. - Signpost patients, families and professionals to appropriate services where direct care is not the right route. - Build strong working relationships with GPs, acute teams, discharge teams and wider health and social care professionals. - Contribute to a coordinated, responsive service that supports people across Cornwall at a critical point in their care.
